Results from a systematic review and meta-analysis assessing the diagnostic accuracy of donor-derived cell-free DNA (dd-cfDNA) for rejection following liver transplantation were published in Transplantation Reviews by Zhang et al. The aim of this study was to evaluate the diagnostic performance and value of dd-cfDNA at different thresholds.
Key data: Overall, 11 studies comprising 901 patients were included. Meta-analyses demonstrated that dd-cfDNA achieved a pooled sensitivity of 0.90 (95% confidence interval [CI], 0.76–0.96) and a pooled specificity of 0.90 (95% CI, 0.82–0.94) for diagnosing rejection. The combined positive likelihood ratio (PLR) was 8.95 (95% CI, 5.39–13.69), with a negative likelihood ratio (NLR) of 0.11 (95% CI, 0.04–0.28). The diagnostic odds ratio (DOR) was 77.39 (95% CI, 35.85–167.03), and the area under the curve (AUC) was 0.95 (95% CI, 0.93–0.97). Heterogeneity was identified across studies in sensitivity (I2 = 76.19%), specificity (I2 = 80.79%), and DOR (I2 = 88.29%).
Key learning: dd-cfDNA demonstrates promising diagnostic value as a complementary, non-invasive biomarker for rejection following liver transplantation. However, heterogeneity in diagnostic thresholds and methodologies highlights the need for further investigation.
